I didn't have any negative experience with the BLS Team. Yes I send in wrong document and they let me know to send the correct document.
After they received my correct document, I got an email that it was accepted and also then email that it was submitted to Consulate.
Word of advise, read instructions very carefully. Take notes and make sure to have cover letter for what you are including in the application. it not only will help them but give you a check list.
I only removed one star is that it would be nice if they included checklist on their site. That would be nice. All US forms have checklist so you are making sure that you are sending correct documents.

Worst Company I have ever seen. Extremely rude and incompetent. Made me wait for more than 1 months before my passport was sent to the consulate. And another 1 and a half months to wait the consulate to process. And totally made me miss my conference in India. Got hanged up by the person pick up my call MANY MANY times. It is clear that they don't want to help you to solve the problem. They just want to finish your call as soon as they can.

This place terrible. The thought of letting this agency handle my important documents is nothing short of harrowing. Their tracking system does not recognize the web file # they themselves issued on a fully completed visa application form. This means I can't event set up an appointment. My experience picking up a visa for my fellow traveller was even more traumatic. The process involves standing in a line that starts in the 4ft wide building lobby at the street level and goes up a stairwell to the 6th floor. If you're disabled and/or old... then it sucks to be you (this has to be some sort of a city code violation). As others have mentioned BLS has hired derelict bouncers who lack any semblance of civility to manage the crowd logistics. BLS deserves credit for having recreated a perfect third world environment in midtown Manhattan, Kudos! Unfortunately there is no other alternative and I have to somehow get through this process and pray they don't lose my passport.

Worst Experience. They are very unorganized, inconvenient, and simply put a very sloppy operation.
I made an appointment and when it was finally my turn after waiting for almost an hour they asked for more random paperwork not stated on their website. I had to come back the next day. They don't take credit cards to pay for the tourist visa. Which is another inconvenience. Also, Obtaining a visa at their Washington DC center takes a minimum of 3-5 days. They don't offer an expedited service which is ridiculous in this day an age. Especially since the Indian Embassy and Consulate are in the same city. My friend in NYC already got his visa and we processed our paperwork on the same day. I could not make my trip and lost money. I understand that this is a third world country but if you are operating in the USA they should meet our standards of processing paper work in a timely manner. You can get a US Passport within hours with supporting documentation (i.e. itinerary) obtaining A TOURIST VISA should not take a minimum of 3-5 business days.
The funny thing is the gentlemen who processed my paper work for my visa made the comment that India needs tourism money, well if they make easier for tourist to travel there maybe we would visit more often. Also manager is useless, but I wont get into that!!! Their customer service is non existent.

Called 9:10 a.m. to confirm hours. Was told to come in between 9am - 5pm.
Double checked all my documents.
Got to the building at 2:10. Was accosted by a giant man smoking out front who claimed BLS was "closed". He was wearing no uniform, just sitting on a chair in front of the building. I insisted on going up anyway.
20 minutes of arguing ensued with the giant security guard and the staff, while 6 beleaguered people stood in line and watched. The guard got the staffer to pull up the website to some hidden page that shows that new visa applications are only accepted from 9 a.m. - 2 p.m.
Reluctantly, I leave, only to have the elevator operator hook me up with the manager, who was coming back from lunch. The manager let me and 3 other people in, because she understood what an inconvenience it was.
That was all just to GET IN LINE!
Then the real horror started. Suffice it to say, when I left, there was a retiree crying in the waiting area, and a young hipster sobbing in the elevator ride down with me. I managed to get my visa application submitted, but God only knows if it will see the light of day.
This place is a disorganized mess. Word to the wise: Do not take "no" for an answer. If they give you a number, ignore it and just go up to the counter. If you sit around waiting for your number to be called, you will never be called! Total horror show. It's like they genuinely don't want people to visit India.
